The Rochester/Monroe County Homeless Continuum of Care Team (CoC Team) was first convened by Monroe County and the City of Rochester in 1994 when HUD announced its homeless continuum of care strategy and is the lead entity for the CoC planning process.

The mission of the CoC Team is to develop, maintain, monitor and continuously improve a comprehensive, flexible and coordinated continuum of care system of services for homeless individuals and families.
